How to get your medical records

In accordance with the Data Protection Act 1988 and Access to Health Records Act, patients have a right to access their medical records. We respect the rights of individuals to have copies of their information wherever possible.

The Practice will continue to be under EU General Data Protection Regulation (GDPR) which came into effect on 25th May 2018.

From 25th May 2018, all patients are required to complete a "Subject Access Request" (SAR) form to enable the Practice to process your request for a copy of some / all of your medical records.

The Practice will endeavour to respond promptly to your request and in any event within 30 days upon receipt of the following:

  • Practice receipt of fully completed SAR form
  • Practice receipt of any further information that we may ask you to provide to enable us to comply with your request.
  • Proof of your identity and of your address to ensure that we are releasing the data to the correct person.
